click on view statement to proceed to
the Consolidated statement page
in the Consolidated statement you may
click here to print or save a copy of
the statement immediately
otherwise a copy of the Consolidated
statement will be available in your
notices and letters within five days
please check to ensure you have filled
in all the information in the tax form
correctly
you may click on go to my tax form if
there are changes to be made
if there are no changes click on proceed
read through the Declaration and once
you are done you may click on submit
once you have successfully submitted
your tax return you will see an
acknowledgment page
click here if you wish to print or save
a copy of the acknowledgment page
